As a per diem weekend tele NP, you'll conduct remote assessments on Saturdays and/or Sundays, act on Circadia platform alerts, and ensure continuity of care for high-risk patients between weekday care team touchpoints. What You'll Do Weekend remote patient care Conduct follow-up and urgent telehealth assessments on flagged patients; adjust plans of care as needed; coordinate with on-call providers and care teams. Data into action Interpret trend-based insights from the Circadia platform, prioritize high-risk patients, and intervene on weekend alerts to prevent unnecessary ED visits or hospitalizations. Close the loop Complete documentation/charts same day via AI-powered tools. Weekend tele-visit management Efficiently complete tele visits for designated home health patients in the SoCal/OC area on weekends, focusing on Circadia alerts and high-acuity flagged patients. Quality & product feedback Surface insights to improve virtual workflows and tech integration in a home health setting. How Success is Measured (KPIs) Visits Target 25+ weekend tele visits/day Flagged patients Time to Acknowledgement Rate less than 24 hours Documentation Complete all notes using AI-powered tools within 24 hours of each encounter. Must-Have Qualifications Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute. Eligible for Medicare participation; company bills on your behalf. 2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently. Comfortable conducting telehealth visits and navigating virtual care platforms. Weekend availability (Saturday and/or Sunday). Thrives in a dynamic, evolving startup environment. English required. Nice-to-Haves Tech-savvy Google Suite, multiple EMRs (HCHB experience a plus), Slack, AI transcription/tools.
