Position Summary We are actively seeking experienced Per Diem Infusion Registered Nurses (RNs) to provide high-quality infusion therapy in patients’ homes across all five boroughs of New York City and Westchester County . This role is ideal for nurses who are flexible, independent, and experienced in infusion therapy. Nurses must be willing and able to travel throughout the service area based on patient needs. You will work directly with specialty pharmacies and Certified Home Health Agencies (CHHAs) to deliver critical clinical care to patients transitioning from hospital to home. Responsibilities • Perform start of care visits and follow-up visits as directed by physician orders • Serve as liaison between pharmacy, CHHA, and care team to coordinate all aspects of patient care • Provide skilled nursing care for infusion therapy in the home setting • Manage and care for central venous catheters (CVC), including PICC lines, Mediports, Hickman, and Broviac • Educate patients and families on infusion therapy and CVC management • Conduct ongoing patient assessments, including dressing changes, blood draws, and care management • Communicate changes in patient condition to the physician promptly • Maintain accurate and timely clinical documentation in accordance with agency policies • Ensure strict adherence to HIPAA and patient confidentiality standards Requirements • Applicants must have at least 3 years of experience as a registered nurse in a home care infusion company, hospital, clinic, or medical facility. • Infusion experience is REQUIRED. • Knowledge of Federal HIPAA Regulations • Knowledge of standard nursing policies, procedures, methods, and techniques. • Ability to read and interpret clinical documents, including physicians' orders and treatment plans. • Ability to participate in a multi-disciplinary health care team. • NYS Registered Nursing License and Registration Certificate • BLS necessary/ACLS preferred • Up-to-date Physical, TB Clearance/QuantiFERON test, 2-step PPD, or Chest X-ray • MMR-Varicella Titers Lab Report or Vaccination Records • Hep B Vaccination Series, Titers OR Declination • Urine Drug Screen (UDS) - within the last year • Flu Vaccination/Declination • Two (2) forms of government-approved ID • Two (2) satisfactory professional references • Reliable transportation • Must have malpractice insurance Nice-to-haves • Fluency in Spanish a huge plus. • Knowledge of principles & practices of supervision, performance evaluation, and education.
